The cost of the Patient Loyalty Program is essentially the same as your annual preventative care (cleanings, exams, and x-rays). Therefore, the program provides a 20% fee reduction to anyone enrolled.

  • Payment is due at the day of the treatment.
  • Plan cannot be combined with other insurance plans, discount plans, or promotions.
  • The Program is for you and is not transferable.
  • Benefits are provided for one year from the date the plan is purchased.
